Diabetes Health Plan

2014 Diabetes Program Classes

-Participants who did not complete all 3 classes in 2013 or newly enrolled members will be required to attend all of the classes offered in 2014 - (1) Introduction to Diabetes, (2) Heart Health and (3) Healthy Lifestyles.

-Participants who did complete all 3 classes in 2013 (Diabetes Overview, Diet, Exercise & Fitness, and Reucing Risk & Prevention) will only be required to complete 2 of the 3 classes in 2014 - (2) Heart Health and (3) Healthy Lifestyles. These two classes will be offered in a combined session throughout 2014 so that some participants will have the ability to complete this requirement in just one day!

Class descriptions and available dates and times are listed below. Choose your class and location, select a date and click register. If the class of your choice does not appear in the drop-down menu, the class has been filled. Please contact the Diabetes Health Plan Administrator, Katherine Bunting, at 667.5959 with questions or concerns. You may also call 815.5259 for assistance.


In 2014, required classes for the Diabetes Health Plan include:

(1) Introduction to Diabetes - Designed as a foundation for all DHP participants, this class provides attendees with an introduction to essential topics related to diabetes management and care. Topics will include understanding diabetes (types and development), medications and treatments, treating a high and low blood sugar, nutrition, carb counting, physical activity, and reducing risk of future complications.

(2) Heart Health - Newly developed for 2014, this class details how heart disease is related to diabetes. Because the two conditions are so closely linked, it is important to understand what tools you can use to prevent future complications. This class will examine pre-diabetes, blood pressure, cholesterol, as well as treatment goals and their relationship to diabetes.

(3) Healthy Lifestyles - Also added in 2014, this class will provide guidance on how to live a healthy, fulfilling life with chronic disease. Topics will include stress and depression management, exercise guidelines and diet.

(4) Diabetes Combo Class - For the convenience of our 2014 DHP participants, this event combines class (2) Heart Health and class (3) Healthy Lifestyles into one single session. See the descriptions above for a content summary of each class.
