Your Care Plan
Your individual case manager, a registered nurse, develops the plan of care that will best meet your needs. Our Home Health Care team of devoted professionals works closely with your physician to monitor your progress and make any necessary changes in your care. For the convenience of the patient, your home care team is equipped with a point of care electronic medical records system that will allow for in-home computerized charting to insure safety, quality, efficiency, and accuracy.
